The Role

You will lead our localities teams, admission avoidance and hospital discharge, integrated mental health, and the Islington Learning Disabilities Partnership.

Our localities social work teams comprise a new Response Team and a Complex Needs team.  The response team will support residents with an existing package of care, linking them to community partners and support networks.  The team will promote our short-term offer including reablement and assistive technology and aim to resolve needs within a maximum of six weeks. The Complex needs team will support people who present with complex needs who have been triaged by our Response Team or Front Door. The team will support all  adults and carers (18+ yrs) with Care Act eligible needs including residents with physical disabilities, long-term health conditions, sensory needs, autism and older adults.

This role will be responsible for the following budget and staff numbers:

Gross Budget: £84,436,181 and 312.28 FTE
